Hi - this isn't PHP/sql but this article (http://www.hybrid6.com/webgeek/2006/12/htaccess-ip-banning-block-bad-visitors.php) might help you, it is done using the .htaccess file.
Or you can do this through cPanel by clicking "Ip Deny Manager" under "Site management" - it allows domains aswell.

Perhaps it is because I am green... but why would one want to throttle what is apparently a major contributor to site traffic? Is it sending bots? ^_^;

Because, everyone from that forum is use my sql connection...
I had a Manual Traffic Exchange, and if member's advertise his/her referral link, s/he get 0.5 point, but one of user paste his link as picture... What is not allowed... and he generate a large traffic (Forum visitor can not see his referral page, beause it is only a picture.....)
And I just wanted to ban this domain...

Maybe you guys should do a PROMINENT sticky about this .htaccess subject.
Post a dummy .htaccess
Include stock and wildcard examples for each use.
Above all, keep it in lay person terms.
For Example:
One of the possibilities I found interesting but haven't yet implimented is the ability to ban by referer.
Tried that coupled with some wildcards and it went bonkers.
None of the .htaccess articles I've read covers anything about wildcards.
